Referrals

All potential clients need to be registered with MY AGED CARE. You are then able to complete an online registration via the MY AGED CARE Portal, and the coordinator is able to view your profile and assess whether or not our service can provide you with the appropriate care and home visiting services (please keep in mind this is a language specific organisation).

What happens after a referral - A date and time for an assessment will be negotiated so we can determine how to best assist you. A carer or a family member must be present during the assessment. A personalised care plan and proposal will be prepared by us for your consideration.
